(“SportGait”, “us,” “we” or “our”) collects through www.sportgait.com, (the “Site”), any other web sites owned or operated by SportGait, our SportGait Parent App application, as well as any other applications owned or operated by SportGait (the “Application”, and collectively with the Site, the “Services”), and how we collect, use, and share that information. This Privacy Statement does not govern our collection of personal information through any other website or other means, other than through the Services. By using the Services, you explicitly accept, without limitation or qualification, our practices surrounding the collection, use, and sharing of personal information provided by you in the manner described in this Privacy Statement. You do not have to give us any personal information to browse the Services. However, you may be asked to provide personal information about you (or if you are the parent of an athlete, personal information about the athlete) to submit or request information from us, or to use the Services. Registration information.When you sign up for the Services, you are required to provide information about you such as your name, username, password, email address, mailing address, telephone number, date of birth, location, and gender. If you are the parent of an athlete, you may also be asked to provide the information about the athlete, including his or her name, mailing address, telephone number, date of birth, location, gender, the sports he/she plays, the number of years he/she has played the sport, and the athlete’s shoe size.
Information about your medical condition.You may choose to provide information about the athlete’s medical condition, including information about the severity of the athlete’s concussion (including the type and severity of the hit the athlete experienced), symptoms, the progress of the athlete’s recovery, and other medical information that you provide.
Additional information.Additional information that you provide to us, including through feedback, messages, emails, answers to surveys or questionnaires that you may submit.
The athlete’s healthcare provider may provide us with information about the athlete so that we may provide the Services to you. This may include, without limitation, information about the athlete’s diagnosis, medical condition, progress towards recovery, and other medical information.
We may collect your physical location information if you have installed the Services on a location-enabled device (including GPS, cellular, and Wi-Fi networks). We collect this information from your mobile device in connection with your use of the Application (as well when the Application is running in the background or if you exit the Application without using “force quit”).
We may collect certain information
about your use of the Services through the use of tracking technologies or by other passive means. This “passively collected”
information includes, but is not limited to, the domain name of the website that allowed you to navigate to the Services,
search engines used, the internet protocol (IP) address used, the length of time spent on the Services, the pages you looked
at on the Services, other websites you visited before and after visiting the Services, the type of internet browser you have,
the frequency of your visits to the Services, and other relevant statistics, including the following:
Log Information. When you access the Services, our servers automatically record information that your browser sends whenever you visit a website.
These server logs may include information such as your web request, IP address, browser type, browser language, the date
and time of your request, your computer operating system, mobile device and mobile operating system, name of your internet
service provider or your mobile carrier, and one or more cookies (small text files containing a string of characters) that

any actions or policies of such third parties.
